Причины, факторы риска, диагностика и лечение желудочно-кишечных кровотечений у больных новой коронавирусной инфекцией (COVID-19)

Аннотация
В статье обсуждаются патогенетические и клинические аспекты желудочно-кишечных кровотечений (ЖКК) у больных новой коронавирусной инфекцией (COVID-19). Показана возможность непосредственного взаимодействия коронавируса с рецепторами ангиотензинпревращающего фермента-2 (АПФ-2) слизистой оболочки желудочно-кишечного тракта (ЖКТ) с развитием ее повреждения и последующего кровотечения. Значение антикоагулянтной терапии как фактора риска возникновения ЖКК оценивается противоречиво. У большинства больных с инфекцией COVID-19 и ЖКК консервативная гемостатическая терапия оказывается эффективной. Влияние ЖКК на течение и прогноз новой коронавирусной инфекции имеет неоднозначную трактовку. Учитывая недостаточную изученность патогенетических и клинических аспектов ЖКК у больных с инфекцией COVID-19, эта проблема требует дальнейших исследований.
