Инфаркт миокарда без обструктивного поражения коронарных артерий (MINOCA)

Аннотация

«MINOCA» (инфаркт миокарда без обструктивного поражения коронарных артерий) — новый термин в кардиологии, который объединил гетерогенную группу патологических состояний, в результате которых развивается инфаркт миокарда при необструктивном поражении коронарных артерий. В статье раскрыты вопросы этиологии и патогенеза MINOCA, которые подразделяются на коронарогенные: необструктивный атеросклероз, коронароспазм, микроваскулярная дисфункция, диссекция коронарной артерии, мышечный мостик, и некоранорогенные: миокардиты, кардиомиопатии, тромбофилии, тромбоэмболии легочных артерий. Привлекают к себе внимание вопросы диагностики и дифференциальной диагностики, что определяет дальнейший выбор тактики ведения и лечения. В настоящее время нет рекомендаций по ведению и лечению пациентов с диагнозом MINOCA, соответственно, не разработаны методы профилактики. Термин «MINOCA» ставит перед нами ряд вопросов, многие из которых остаются открытыми для дальнейшей дискуссии и решения.
