В то же время все большая доля больных требуют сочетанных хирургических вмешательств в дополнение к протезированию аортального клапана. В настоящее время оперативная летальность при изолированном аортальном стенозе варьирует в пределах 3–8% у пациентов низкого риска моложе 70 лет и между 5 и 11% у «возрастных пациентов», достигая 15% в случаях сочетанных вмешательств на сердце. Применение бесшовных клапанов для открытой имплантации требует тщательного анализа ближайших и отдаленных результатов их использования и сравнения с результатами стандартных методов протезирования. </p><sec><title>Материал и методы</title><p>Материал и методы. Разработанный дизайн исследования представляет собой сравнительное проспективно-ретроспективное исследование по оценке вмешательств с использованием разных видов биологических протезов при поражении аортального клапана в сочетании с сопутствующей кардиальной патологией и без таковой. В исследование были включены пациенты старше 65 лет с аортальным стенозом, которым было выполнено протезирование аортального клапана. Пациенты были разделены на две группы. Сравнение проводилось по следующим показателям: интраоперационные показатели, количество и характер послеоперационных осложнений, выживаемость, свобода от ишемических событий и от репротезирования, динамика трансаортального градиента, гемодинамические характеристики за весь срок наблюдения (5 лет). </p></sec><sec><title>Результаты</title><p>Результаты. В результате в группе пациентов с имплантированным бесшовным клапаном отмечалось меньшее количество п/о осложнений, быстрейшее восстановление, достоверно более низкие показатели смертности, более высокая свобода от клапан-связанных осложнений как в ближайшем, так и в отделенном послеоперационном периодах. </p></sec><sec><title>Выводы</title><p>Выводы. Применение бесшовных протезов вполне оправдано для протезирования аортального клапана и более безопасно по сравнению со стандартными вшиваемыми при отсутствии противопоказаний. Хирургическое лечение пациентов с аортальным стенозом с применением бесшовных клапанов привело к значительному клиническому улучшению у большинства из них как в функциональном классе, так и в пороге толерантности к физической нагрузке, а уровень характерных осложнений оказался достоверно ниже. Простота и воспроизводимость самой процедуры, быстрый процесс обучения, безусловно, могут способствовать более широкому и активному внедрению данной технологии в клиническую практику.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: aortic valve replacement is a «gold standard» in the surgical treatment of aortic stenosis. At the same time, an increasing number of patients require combined surgical interventions in addition to aortic valve replacement. Currently, surgical mortality in isolated aortic stenosis varies between 3–8% in low-risk patients under 70 years old and around 5–11% in elderly patients, rising up to 15% in cases of combined heart surgery. The use of sutureless valves for open implantation requires careful analysis of the immediate and long-term results of their implantation and comparison to the results of standard methods of treatment. </p></sec><sec><title>Material and methods</title><p>Material and methods. The study was designed as a prospective-retrospective study for evaluation of results of diﬀerent types of biological prostheses use in case of isolated aortic valve stenosis and together with concomitant cardiac pathology. The study included patients over 65 years old with aortic stenosis, who underwent aortic valve replacement. The patients were divided into two groups. The comparison was carried out by the following criteria: intraoperative data, the number and nature of postoperative complications, survival, freedom from ischemic events and from reoperation, dynamics of transaortic gradient, hemodynamic characteristics for the entire evaluation period (5 years). </p></sec><sec><title>Results</title><p>Results. In the group of patients with implanted sutureless valves, there was a smaller number of postoperative complications, faster recovery, significantly lower mortality rates, higher freedom from valve-related complications in postoperative period.</p></sec><sec><title>Conclusions</title><p>Conclusions. The use of sutureless prostheses is justified for the aortic valve replacement and is safer compared to standard valves, in the absence of contraindications. Surgical treatment of patients with aortic stenosis with the use of sutureless valves led to significant clinical improvement in most of them, both in the functional class and in the threshold of tolerance to physical activity. The level of typical valve replacement complications was significantly lower.
