Обследованы 158 пациентов со стабильной ХСНсФВ. Диагноз ХСНсФВ устанавливали при наличии симптомов и/или признаков ХСН, ФВ левого желудочка 50%, повышения уровня предшественника мозгового натрийуретического пептида (NT-proBNP) крови и характерных структурных изменений сердца. В исследование не включали пациентов с первичным гиперальдостеронизмом и лиц, принимавших антагонисты минерало-кортикоидных рецепторов в течение предшествующих 6 нед. У всех пациентов определяли плазменные концентрации альдостерона и NT-proBNP.</p></sec><sec><title>Результаты</title><p>Результаты. Средний возраст пациентов составил 69,9 ± 6,35 года, среди них преобладали женщины (64%). У 59 (37%) больных уровень альдостерона крови превышал верхнюю границу референсного диапазона (160 пг/мл), у остальных 99 (63%) он был в пределах нормы (40–160 пг/мл). Пациенты с гиперальдостеронемией были моложе и чаще страдали сопутствующей патологией. Они имели более высокий уровень NT-proBNP по сравнению с пациентами с нормальной концентрацией альдостерона плазмы (медиана 480 (356–623) пг/мл против 224 (165–302) пг/мл, р &lt; 0,001). Показатели индекса массы тела, систолического артериального давления, общего холестерина, липопротеинов низкой плотности, креатинина и выраженность диастолической дисфункции (Е/е’) у пациентов с гиперальдостеронемией были достоверно выше, а уровень калия и скорость клубочковой фильтрации — ниже по сравнению с группой нормальной концентрации альдостерона (все р &lt; 0,05). Корреляционный анализ выявил наличие мощной высокодостоверной связи между уровнем альдостерона и NT-proBNP (r = 0,593, р &lt; 0,001).</p></sec><sec><title>Вывод</title><p>Вывод. У больных ХСНсФВ плазменная концентрация альдостерона тесно взаимосвязана с уровнем NT-proBNP. Уровень NT-proBNP существенно выше среди лиц с гиперальдостеронемией по сравнению с больными, имеющими нормальный уровень альдостерона. По мере возрастания степени гиперальдостеронемии плазменная концентрация NT-proBNP увеличивается.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose. The study aims to investigate the interrelation between plasma levels of aldosterone and brain natriuretic peptide in heart failure with preserved ejection fraction (HFpEF).</p></sec><sec><title>Material and methods</title><p>Material and methods. A cross-sectional study included 158 patients with HFpEF. The diagnosis of HFpEF was made in the presence of symptoms and/or signs of heart failure, left ventricular EF 50%, increased NT-proBNP level and structural changes in the heart within echocardiography. The study did not include patients with primary hyperaldosteronism and those who received mineralocorticoid receptors antagonists in the previous 6 weeks. In all patients plasma levels of aldosterone and NT-proBNP were evaluated.</p></sec><sec><title>Results</title><p>Results. Patients’ mean age was 69.9 ± 6.35 years; most of them were women (64%). According to laboratory results 99 patients (63%) had normal (40–160 pg/ml) aldosterone plasma level (nAld) and 59 patients (37%) had high (160 pg/ml) aldosterone level (hAld). hAld patients were younger and, most likely, had comorbidity. They had higher level of NT-proBNP compared to nAld (median level 480 (356–623) pg/ml versus 224 (165–302) pg/ml, p &lt; 0.001). Body mass index, systolic blood pressure, total cholesterol, low density lipoproteins, creatinine and E/e’ were signifi cantly higher in hAld group, while plasma potassium and glomerular fi ltration rate were lower compared to nAld group (р &lt; 0.05). Plasma aldosterone was strongly and positively correlated with NT-proBNP (r = 0.593, p &lt; 0.001).</p></sec><sec><title>Conclusion</title><p>Conclusion. In HFpEF plasma aldosterone is closely related to NT-proBNP.
