Сложность этого диагноза обусловлена тем, что его можно поставить только ретроспективно, когда после успешного контроля ритма, включая кардиоверсию или абляцию, восстанавливается систолическая функция левого желудочка и уходит клиническая симптоматика. Рассмотрены вопросы распространенности заболевания и его место в современной классификации кардиомиопатий. Отмечены особенности структурных, функциональных и гемодинамических изменений в сердце. Обсуждаются современные подходы к диагностике заболевания и выбора эффективной стратегии лечения.</p></abstract><trans-abstract xml:lang="en"><p>The article presents a clinical case of a diagnostically difficult disease – arrhythmogenic cardiomyopathy in a patient with paroxysmal atrial fibrillation. The complexity of this diagnosis is due to the fact that it can only be made retrospectively, when after successful rhythm control, including cardioversion or ablation, the systolic function of the left ventricle is restored and clinical symptoms disappear. The issues of the prevalence of the disease and its place in the modern classification of cardiomyopathies are considered. The features of structural, functional and hemodynamic changes in the heart are noted.
