В настоящее время в России, несмотря на внедрение фиброэндоскопической методики, основным методом радикального лечения остается открытая операция – дивертикулэктомия с миотомией нижней порции крикофарингеальной мышцы и пищеводной миотомией.</p><p>Целью исследования явилось улучшение результатов лечения дивертикула Ценкера путем совершенствования хирургической тактики. Сравнительному анализу подвергнуты результаты обследования и лечения пациентов с дивертикулом Ценкера, которые были прооперированы в объеме открытой дивертикулэктомии с крикофарингеальной миотомией (21 наблюдение) или трансоральной эзофагодивертикулостомией (19 наблюдений). Были проанализированы: возраст пациентов на момент диагностики заболевания и на момент операции, предъявляемые жалобы, количество и характер сопутствующих патологий, размер дивертикулярного мешка, осложнения основного заболевания, длительность койко-дня, предоперационного и послеоперационного периодов, время операции, результаты лечения (исход, наличие рецидивов в отдаленном послеоперационном периоде). Показано, что трансоральная эзофагодивертикулостомия не уступает по эффективности открытой классической операции, является малотравматичной методикой с минимальной вероятностью возникновения интра- и послеоперационных осложнений, может быть применена у пациентов с рецидивными дивертикулами и после операции на позвоночнике из переднего доступа.</p></abstract><trans-abstract xml:lang="en"><p>The choice of surgical treatment for patients with Zenker's diverticulum has been the subject of heated debate for many decades. Currently, the main method of radical treatment in Russia remains open surgery – diverticulectomy with myotomy of the lower portion of the cricopharyngeal muscle and esophageal myotomy. The purpose of the study was to improve the results of treatment of Zenker's diverticulum by improving surgical tactics. The results of examination and treatment of patients with Zenker's diverticulum who were operated on with open diverticulectomy with cricopharyngeal myotomy (21 cases) and transoral esophagodiverticulostomy (19 cases) were subjected to comparative analysis. Were analyzed: age of patients at the time of the time of diagnosis and the time of surgery, complaints, number and kind of concomitant pathology, size of the diverticulum pouch, complications of the main disease, hospital length of stay, preoperative and postoperative periods, results (outcomes, relapses in the long-term postoperative period).
